What do I need to bring to my appointment? | Lemak Health

  1. A photo ID
  2. Your Insurance Card
  3. A list of your medications
  4. ALL previous medical records regarding your injury (including imaging films and paper reports)

If your insurance requires a copay, you will be required to pay at the time of service

You must cancel your appointment 24 hours before your scheduled appointment, or you will be charged a fee of $25.00

Please give our staff at least 24 hours to have a written prescription ready from your provider.

We will only be able to discuss your medical records with the people you put on your paperwork. If a person is not listed on your paperwork, we will not be allowed to discuss any information at all
